Objective: to analyze the difference in blood glucose level during the acupuncture intervention combined with the metformin 500 mg tablet and the intervention with the metformin tablet alone metformin 500 mg in type 2 diabetes mellitus. Methods: Randomized control study with a six-week follow-up was performed. This research was a randomized pre-post test. A total of 85 patients with type 2 diabetes mellitus underwent acupuncture at the private clinic “Rungkut” in Surabaya, Indonesia, from January 2013 to January 2014. After screening, 73 subjects who met the inclusion criteria were invited to participate in the study. 13 subjects dropped out because they had stopped treatment, because they were too busy to participate due to conflicts in working hours, family problems, feeling unwell, or moving to another city. The total sample was 60 patients divided into 2 groups, control group and treatment group, each consisting of 30 patients. The control group is administered a 500 mg metformin tablet 2 times a day for 6 weeks. The treatment group is administered metformin 500 mg tablet 2 times a day and acupuncture three times a week for 6 weeks, using sterile Huanqiu acupuncture needles, specifications 0.25 x 40 mm (1.5 cun). The acupuncture point principle uses Zusanli (ST36) Position 1 finger lateral to the lower end of the tuberosity of the tibia. Puncture – 1 cun perpendicularly and Taibai (Sp 3) Location: posterior and inferior to the head of the first metatarsal bone at the border of two skin colon. Both group activities, exercise and diet as usual without changes. Age data analyzed using the frequency test; gender data using Fisher's exact test; education data using crosstab testing; the duration of diabetes data using the descriptive test; correlation age and fasting plasma glucose (FPG)... the focus of the paper... the rol level is -14.17 mg/dl, p=0.001. According to the standard efficacy of traditional Chinese medicine for diabetes in the treatment group: healing was 10%, that is, 50% effective, 40% ineffective, while the control group: 6.67% healing is, 33.33% effective, 60% ineffective. The overall success rate in the treatment group was 60% and in the control group 40%. Standard effectiveness of TCM, comparison between the treatment and control groups showed that both were equally effective, meaning that the symptoms improved after treatment, but in this case it is more effective in the treatment group by 20% in more than the control group. Conclusion: the use of acupuncture points combine medicine in this research using Metformin 500 mg tablet twice a day, there is a significant difference in the therapeutic magnitude of the decrease in total cholesterol level p = 0.001 although for l extent of decrease in fasting
