History of Current Illness This was his second episode since 10 days ago in which he developed the same pain in his right side. He suddenly felt severe pain 8 hours before admission when the pain moved to the right lower quadrant of the abdomen. The onset occurs at 6.30 am, and then worsens between 10.00 pm and 2.00 pm. He describes the pain as a sharp, continuous pain that gradually increases in intensity. There is no pain radiation. The pain was exaggerated on movement and touch. There were no relief factors and it scaled in severity as 7/10. He had fever for 1 day before admission. It was a continuous mild fever. He did not experience thrills and rigors. The patient does not experience nausea or vomiting, no dysphagia, no pain during urination and no alteration of bowel habits. He experienced loss of appetite but did not notice any weight loss. Systemic reviewCentral nervous system: He has no paresthesia and has no experience of loss of consciousness or seizures. He has no limb weakness and no hearing problems or visual disturbances. Cardiovascular system: Does not experience any chest pain or palpitation. He has no dyspnea or leg swelling. Respiratory system: no shortness of breath, cough and haemoptysis. Genitourinary system: he has no pain during urination and no increase in urination and does not need to urinate. getting up in the middle of the night to urinate. There is no blood in the urine. Musculoskeletal system: does not present stiffness and swelling in the joints. He can walk normally and has never had a history of falls. Endocrine system: It has no intolerance to heat and cold. He is not excessively thirsty. Other: He has no rash, bleeding or bruising anywhere on his body. Past medical history. This is his first admission to the ward after having the second episode of the same pain. The first episode occurred 10 days before his hospitalization. The first episode resolved after taking painkillers in the clinic. Previous surgical history The patient has no previous surgical history. Drug history He is currently not taking any medications.
