General Appearance My client is a 25 year old South Asian female. She appears slim with long black hair and brown eyes. He wears dark pants, a long-sleeved blue shirt with a multicolored scarf. His clothing is neat and appropriate to his environment. She wears makeup, looks clean and is well put together. The customer answers direct questions but does not provide much information. The patient is willing to provide additional information when asked directly or solicited by the writer. The client's speaking rate was slow at first, but as the interview continued, it became normal. His speech was clear; however he spoke in a low voice, in a monotone. His pace was smooth and his speech was not pressured. The client's energy level appeared low and during the interview he sat with a rigid posture and was very still. The client maintained focused eye contact with the writer throughout the interview. His body language showed no signs of emotion and his facial expression appeared sad and sometimes bored towards others. Emotional state The client described her mood as "very sad and unhappy." He rated his mood on a scale of one to ten, with one being described as the worst you've ever felt and 10 being the best, like a three. When asked to rate her mood upon entering the hospital on the same scale, she rated her previous mood from one to two. He acknowledged that he had started to feel a little better, but his mood was still low. He felt his mood improved thanks to the medications he took and the presence of more people. The customer also had a flat effect. When the client told the writer about the breakup with her ex-boyfriend, there were very few indications of feelings or emotions... halfway through the paper... the interview up to that point was over, I could have clarified and I reflected more about his responses, but I clarified how often he felt auditory hallucinations at 4:58 of the video. I prepared for this task and believe that, through my investigative skills and active listening, I effectively assessed my patient. One area I plan to focus on in the future and improve is my anxiety when interviewing my patients. If I were less anxious, I wouldn't get so nervous and would eliminate some awkward sentences to make my speech flow more smoothly. Even though I failed to ask about illusions, I believe I have gathered relevant and important information. The information I have collected would be useful to the healthcare team because they could then customize the interventions and treatments that would best suit my patient and improve the overall care in which she would receive.
